  • WHY ARE SOME HOSPITAL FORMS UNAVAILABLE FOR VIEWING?
  • Unavailable hospital forms are in inventory but haven't been cleared for publication. As many hospital forms are sourced from medical litigation, patient data must first be erased before forms can be cleared for web publication. For content-dense forms, digitally erasing patient data is an especially labor intensive process.


  • WHAT IS THE DIFFERENCE BETWEEN 'TYPESET', 'SCANNED' & 'BACKORDER' FORMS?
  • Typeset forms are the highest image-quality forms on this site. Scanned forms are the lowest image-quality forms on this site, and often originate from closed medical litigation cases. Backorder forms are in inventory but haven't been cleared for publication.


  • DOES HOSPITAL-FORMS.COM SELL PAPER FORMS?
  • NO.  We custom-build electronic electronic hospital-forms systems for institutional-grade clients. For smaller sized clients + individuals we offer forms typesetting services. If you find a form on this site you wish to have professionally printed, we recommend using Vista Print.


  • WHY DO 3 BANNERS DISPLAY AT TOP OF PAGE (INSTEAD OF 1)?
  • Typically, it's because of 1 of 2 reasons:

    1st, it might be because you're using Internet Explorer and Compatability Views is set to "off".
    To fix this, click the "Tools" dropdown (located in top left corner), then "Compatability View", and the banner should reload properly.

    2nd, it might be because your browser settings are blocking script that needs to run for website to function properly.
    If you see a button at the bottom that says "Allow blocked content", click the button and the banner should display properly.

    ** = Forms obtained from hospitals who had received Joint Commission accreditation at time of initial publication. It is possible that hospitals whose forms reside on this site have since lost their accreditation.
